1. All dogs must be fully vaccinated for DHPP, Rabies and Bordetella (within the past 6 months). Vaccination documentation is required.
2. All new clients must create an account using our online enrollment system.
3. A valid credit card must be posted on your account before a reservation can be made.
4. All new clients are required to sign an online release form.
Daycare and Boarding Requirements:

Follow these steps to become a "New Client"
Daycare and Boarding
1. Select our New Client link on this page to create your account.
2. Request a "Doggie Interview" online or by calling 801-261-2665. All requests submitted online with be confirmed by our staff by phone within 24 hours.
3. Visit DogMode at scheduled time to meet our staff, present most recent vaccination records, view our facility and test your dog's compatibility for our open environment.
4. Complete FREE trial day before any boarding or daycare reservations.
Aquatic Center
1. Select our New Client link on this page to create your account.
2. Request a "Swim Orientation" online or by calling 801-261-2665. All requests submitted online with be confirmed by our staff by phone within 24 hours.
3. Visit DogMode at scheduled time to meet our staff, present most recent vaccination records, view our facility and test your dog's ability to swim.
Grooming
1. Select our New Client link on this page to create your account.
2. Request a "Grooming appointment" online or by calling 801-261-2665. All requests submitted online with be confirmed by our staff by phone within 24 hours.
3. Visit DogMode at scheduled time to meet our staff, present most recent vaccination records, view our facility and complete a grooming consultation with one of our groomers for your appointment.
